UPDATE: Lieutenant Troy Dangerfield tells SF Weekly that witnesses said the shuttle driver ran a red light.

Yesterday, we delivered the sad news about Kevin Mack, a UCSF psychiatrist who was killed in a major car crash. Mack was on his way to work when a big rig collided with the shuttle he was riding in, killing the doctor instantly.

The 52-year-old was thrown from the UCSF shuttle and pinned under the big rig -- and perhaps this is why: The 15-passenger shuttle had no seat belts.

"Seat belts are not required in vehicles meeting the definition of a bus," CHP spokeswoman Jaime Coffee told the Bay Citizen. The CHP, which inspects the campuses' shuttles, had given the bus a satisfactory rating prior to the crash.

The UCSF shuttle was headed northbound on Octavia Street en route to San Francisco General Hospital when it collided with a big rig traveling eastbound on Oak Street. Three other passengers were injured, but are listed in fair or good condition, according to news outlets.

Mack served as a U.C. San Francisco associate professor and also worked in General Hospital's psychiatric emergency room. He leaves behind his husband and two children.

The circumstances of the accident is still being investigated.
